Welcome to the Boston Child Health Survey

By participating, your household is doing a valuable public service by helping to guide public health policies and prevention programs that serve children in Boston.

As noted in the letter your household received, the survey is designed to be completed by the parent or guardian who knows the most about the health of the child or children in the household. Please ask the member of your household who knows the most about the health of the child or children in the household to complete the survey.

The eligible adult who completes the survey will receive a $10 Amazon gift code immediately after completing and submitting the questionnaire, in appreciation for their participation.
